To be purchased over the counter under medical supervision.

Cow’s Milk Allergy: Soya Formula: NO LONGER AVAILABLE

Gastro-oesophageal reflux disease (GORD) Thickened Formula: Aptamil Anti-reflux®, Cow and Gate Anti- Reflux®, SMA Anti- Reflux®

Lactose-free Formula: SMA LF, Aptamil Lactose-Free. For use in managing symptoms of secondary lactose intolerance. Not suitable for infants with suspected cow’s milk allergy. Should only be used for a maximum of 6-8 weeks after which infant should be challenged with a standard cow’s milk based infant formula. 

After 1 year can use shop bought full fat lactose free milk.

Extensively hydrolysed formula (EHF)

Formulary

For use first line if mild to moderate cow's milk allergy suspected

Birth to 1-2 years

SMA Althera (Whey-based) Whey-based formula contain lactose.

Aptamil Pepti 1 (Whey-based) Whey-based formula contain lactose.

Nutramigen 1 LGG (Casein-based) Use casein-based first line in infants with severe diarrhoea/severe GI symptoms who would benefit from a lactose free formula

Hydrolysed rice formula (HRF) (ArizeĀ®)

Second line choice: For use with treatment for symptoms of a cow’s milk allergy if symptoms have not improved after use of an EHF following a 2-4 week trial

To be started in secondary or specialist care if an infant is at risk of or has been diagnosed with faltering growth.

Birth to 18 months or > 8kg of body weight

For use in pre-term neonates. Initiation via dietitian.

Red For Beds/Luton: Hospital only prescribing until discharge supply pathway developed.

Amber SPIS For Milton Keynes: GP may prescribe the remainder of the course when provided with full information from dietician via template letter.

Preterm post discharge formula

Formulary

To be started in secondary or specialist care only. For infants under 2kg birth weight AND less than 34 weeks gestation.

From birth to 3-6 months corrected age

NB: Liquid preparations are not routinely recommended for use in the community due to cost, unless under the care of a dietitian and there is a specific indication

Specialist to advise therapy and provide first 28 days supply, continuation in Primary Care
Specialist to initiate and stabilise medicine prior to continuation in Primary Care
To be prescribed as per Shared Care Guidance. If no SCG in place status reverts to red.
Red medicines are designated as specialist only medicines which should only be prescribed by a specialist, usually within secondary care (either due to the requirement for specialist knowledge, long-term monitoring requirements, or restrictions that mean medicine supplies are only available to hospitals).
A decision has been made either locally and/or nationally not to routinely commission this preparation. Do not prescribe.
To be purchased over the counter. May be prescribed for chronic, long term conditions or on admission to hospital if essential.
