Vancomycin capsules should be used as first line treatment of C.difficile for all adult patients where oral administration is appropriate

There are some situations where the IV formulation can be administered via the enteral route:

-          Administration via enteral feeding tubes (unlicensed)

-          Patient with swallowing difficulties (unable to swallow capsules but able to swallow liquids)

Vancomycin oral
(Capsules)

Capsules 125mg , 250mg

Capsules can be used to treat C. difficile in patients being discharged from hospital, or in Primary Care in line with the BLMK Antimicrobial Prescribing Guidelines.

Vancomycin capsules should be used as first line treatment of C.difficile for all adult patients where oral administration is appropriate

There are some situations where the IV formulation can be administered via the enteral route:

-          Administration via enteral feeding tubes (unlicensed)

-          Patient with swallowing difficulties (unable to swallow capsules but able to swallow liquids)

Monitor blood levels if appropriate.

at following concern about the availability of oral vancomycin in primary care was raised at the Antimicrobial steering group, an agreement is now in place to ensure the following pharmacies will hold at least 28 x 125mg vancomycin capsules in stock at all times.

Specialist to advise therapy and provide first 28 days supply, continuation in Primary Care
Specialist to initiate and stabilise medicine prior to continuation in Primary Care
To be prescribed as per Shared Care Guidance. If no SCG in place status reverts to red.
Red medicines are designated as specialist only medicines which should only be prescribed by a specialist, usually within secondary care (either due to the requirement for specialist knowledge, long-term monitoring requirements, or restrictions that mean medicine supplies are only available to hospitals).
A decision has been made either locally and/or nationally not to routinely commission this preparation. Do not prescribe.
To be purchased over the counter. May be prescribed for chronic, long term conditions or on admission to hospital if essential.
